As someone who's worked in healthcare for 20+ years, I completely support the idea of having things written out beforehand and giving copies to all the providers.  Specifically tell them you "want it put in your medical record". (that covers written and electronic both). having stuff in writing will always outweigh hearsay and will weaken any "I didn't know" arguments. Then the onus is on them to have read it. The multiple copies is also a CYA kind of thing just in case someone does make an honest mistake and misplaces it.

I also agree with Kizzie about asking the nurse to tell recovery. It's a separate area and you want to ensure your wishes are communicated. If you're in the USA, the shift changes should be telling each other as well.  Normal hospital shifts are either 7a-3p, 3p-11p, 11p-7a, or they might have some people on 12-hour shifts that are either 7-7 or 11-11.  The day/swing/night is more common in surgery/recovery, though.
